TUBERCULOSIS (TB) + EMPLOYEE HEALTH

The Clinical Services Division is accepting applications for a Community Registered Nurse II (Community RN II) to work in an office or community outreach setting and interact regularly with key stakeholders and/or the medical community to affect enforcement of complex federal, state, and/or local mandates regarding public health nursing matters while also delivering and/or coordinating a full range of quality public health nursing services essential for promoting and protecting the safety and health of the Northern Kentucky community, and to perform related duties, thereby promoting a safe and healthy community. Initially the position will be assigned to the Tuberculosis (TB) and Employee Health programs and will be located at our District office currently at 8001 Veterans Memorial Drive in Florence KY. Status: Full-time (40 hours/week) non-exempt (hourly pay) merit system position

Starting Pay Rate: $33.77-$41.44/hour (based on your years of directly related paid experience)
Reports to: Programs Manager

Essential functions include, but are not limited to:

  1. Coordinates referral services for assigned program(s). (TB: Identifies community resources for TB referrals. Maintains list of referral sources for assigned program(s) and disseminates as appropriate. Makes referrals to community resources. Reaches out to clients who were referred. Schedules appointments with clients. Employee Health: Coordinates post-exposure care of staff with BBP exposure.)

  2. Performs client services for assigned program(s). (TB: Identifies and collects physiologic, psychosocial, and environmental data by interview and/or physical examination to detect and identify problems for TB clients. Performs health assessments and screenings by interview and/or physical examination. Develops plans of care for TB clients to achieve expected outcome(s) by identifying problems, identifying needed resources and works with client and/or client medical providers to develop a timetable for reaching goals. Conducts case reviews for all active TB and Latent Tuberculosis Infection (LTBI) clients. Follows up with clients at regular intervals to determine level of compliance with established plan of care and to make adjustments where necessary. Shares findings from follow-ups as needed. Provides client education in an individual or group setting. Acts as liaison between the physicians treating TB patients, nursing, and designated case management staff. Notifies other Health Department staff of client’s needs. Employee Health: Provides in stock recommended vaccination and/or TB testing for staff as appropriate for established category. Provides post-exposure counseling to staff with BBP exposure. Provides fit testing and re-testing for respirator use.)

  3. Serves as resource person to Health Department and community for assigned program(s). (Collaborates with community partners. Ensures any grant or other mandated requirements for assigned program(s) are met. Maintains communication with other disciplines in coordinating health care activities. Educates Health Department staff regarding changes/advancements in assigned subject matter. Provides general technical assistance and consultation to schools, health professionals and organizations, co-workers, community organizations, and the general public on designated public health service(s) or program(s). Answers questions about protection from and exposure to Blood Borne Pathogens (BBPs). TB: Assists with developing and maintaining lines of communication and relationships with local care providers for TB clients to facilitate the coordination of comprehensive care and support between the medical home, public health, and community agencies. Acts as a CPR trainer for the Health Department.)

  4. Performs community outreach activities to raise awareness of assigned program(s). (Coordinates internal and external committee/team meetings by scheduling meetings/team activities, establishing agendas for meetings, and handling pertinent communications. Assess and define priority populations to provide outreach activities and linkage to other needed services Prepares and makes educational presentations target groups and individuals. Prepares and distributes educational materials to community and healthcare facilities. Assists with offsite clinics as needed. Develops TB Screening and Outreach program. Monitors TB Hotline and initiates follow up according to established standards. Monitors Electronic Disease Notifications (EDN) and initiates follow up according to established standards. Coordinates TB notification and education to client’s employer, landlord, family, friends, community partners, etc.)

  5. Ensures compliance of assigned program(s). (TB: Identifies individuals and/or entities in the medical community who are key stakeholders then works with them to ensure that they are in compliance with federal, state and/or local mandates affecting assigned program(s). Conducts on-site reviews and audits of TB and Employee Health programs. Prepares reports of TB program reviews and audits. TB Maintains, monitor, and audits client charts. Communicates problems with care or charts to staff. Employee Health: Monitors compliance with established workplace health standards, attempts to correct any non-compliance, and notifies supervisor of continued non-compliance. Reviews TB testing for staff. Investigates any conversions. Conducts surveillance of Health Department staff for TB infection and disease.

  6. Assists with management of assigned program(s). Assists with developing program goals, objectives, and guidelines necessary for effective implementation of services to target population. Assists with evaluating program effectiveness and developing plans for increasing effectiveness in the future. Assists with community needs assessment and identifying resources to meet those needs. Serves as program owner for grant submissions as assigned. Ensures that established protocols are compliant with current laws and best practices or recommends specific changes to appropriate staff as needed. Employee Health: Reviews and recommends updates to the following sections of the Safety Manual: Section 5: BBP Exposure Control Plan, Section 6: Prevention of TB Transmission Plan, and Section 7: Hazard Assessment and Personal Protective Equipment. Worksite Wellness Acts as the Worksite Wellness Coordinator overseeing the planning, implementation and evaluation of wellness programs. Reviews and recommends updates to the Worksite Wellness Policy as needed.

  7. Maintains records and produces reports. (Maintains client medical records including records of staff exposures to BBP. Maintains database(s) of case information.)

  8. Completes special projects. (Responds to public health emergencies as assigned; Completes other special projects.)

  9. Performs other duties. (Attends work as scheduled or uses approved leave. Collaborates in and contributes to individual, team, and/or organizational quality improvement and evaluation activities. Participates in internal and external meetings. Serves on internal and external committees. Completes timecard. Completes travel reports. Completes required training. Completes other assigned reports. Attends work as scheduled or uses annual leave.) 

Minimum Qualifications:

Valid, Active Driver’s License, license to work as an RN in KY, CPR certification, and one of the following:

  • Successful completion of Introductory Period as a Community RN and demonstration of mastery of competencies sufficient to perform the essential functions of the job.
  • Equivalent education as determined by licensing board recognized by the Kentucky Board of Nursing PLUS the equivalent combination of experience sufficient to successfully perform the essential functions of the job.

 

Desired Qualifications:

Same as above plus prior public health experience, experience with TB and/or Employee Health, and/or bilingual English/Spanish.
